1) The address you called the taxi to (pick-up address)
2) The address you went to (destination address)
3) The date you called the taxi (Please remember 'after midnight calls' are actually the NEXT day.)
4) The time you called the taxi
5) If it was a 'public place' (bar, restaurant, hotel, etc.), the name you gave to the operator
6) If you know the car number of the Co-op Taxi that picked you up, that is of course very helpful
7) If you wish to be contacted by phone, a contact name and phone number
